Screaming in pain, nothing to worry about at all. How numb has this doctor gotten that a patient screaming doesn’t immediately alert them that it’s not “nothing to worry about.” For a pinched nerve, resting is certainly the best way to let it recover naturally, but sometimes a brace or splint to hold the damaged area is needed.
In any case, if someone is in pain, the first instinct should not be to immediately trigger that pain as severely as possible. Isn’t there a process to it? Maybe a bit of prodding and poking and gradually seeing where the starting point is?
